The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

  
Before treatment is performed, we will be happy to discuss treatment and finical options. This will help you to fully understand your dental treatment, what to anticipate in fees, and allow you time to make the necessary financial arrangements. No Show/Cancellation Policy

A “No Show” is when a patient fails to appear for a scheduled appointment without providing a 24-hour cancellation notice. There is a $75 charge for all failed appointments. This charge cannot be billed to insurance and must be paid on or before the next scheduled appointment.


To assist you in keeping appointments, we will initiate a reminder text or telephone call before each scheduled appointment. 


It is the responsibility of the patient receiving the message to confirm, cancel or reschedule 24 hours before the scheduled appointment.

 

Late Arrival Policy

Late arrivals of more than 10 minutes without notice, will be considered a “No Show” and will be charged accordingly.
